Staff Availability - Extra Shifts

A  payroll memo has been distributed with this week's pay check.    A copy can be found under "Payroll Memos" dated June 17, 2010. 
 
Please note an amendment to the availability form, effective this coming week.  All Permanent Part Time and Full Time staff are asked to confirm if they only wish to be called for shifts at their program, or if they wish to be called for any shift throughout the agency.  Please check the appropriate box at the bottom of the availability form.
 
COMPLETING AVAILABILITY FORM
In order the complete the form below, you must first open the form and then save it to your computer.  Once it is saved, you may complete it, save it again, and then submit it to tceoncall@tceottawa.org
 
Please ensure to fill in your name and ONE contact phone number on your form prior to submitting.
 
 

Should your availability change after you have submitted for the following week, we encourage staff to e-mail tceoncall@tceottawa.org notifying On Call of the changes.
